Electronic Reserve Request Use this form to request that the library place your e-reserve materials in the Content Collection segment of MyUSF (Blackboard)
Media Resources - Scheduling Services Faculty may schedule media materials and/or multi-media rooms for specific lecture dates, and students may schedule media materials for class presentations, by completing this form.
Reserve Request for Media Faculty may complete this form to request that Media Resources materials (or your personal copies of materials) be placed on one of three types of Reserve: 3-hour (room use only), 24-hour, or 3-day. Please allow 48 hours for your request to be processed.

Books, Journals, Media (Circulation problems, questions, etc)
Forms here relate to issues with book and media circulation.
Book Search: Request help finding a book in the Library USF faculty, staff, and currently enrolled students may use this form inside the library to request a search for a book that is either:
currently listed in the Library Catalog as "Not Checked Out" but which is not on the shelf; or
currently charged to you, but is under dispute.
Please Note: This form is not accessible from outside of the Library. You must use a Library computer to use this form.
Book Search: Search Request USF faculty, staff, and currently enrolled students may use this form to request a search for a book that is currently charged to you, but is under dispute.
Lost Book Use this form to declare that you have lost a book charged to your ID.
Online Renewal Problems Use this form to renew your books if you are having trouble renewing using the self-renewal because of:
fines
overdue books
over the book limit
problems with your USF ID
Periodical/Journal Search Request USF faculty, staff, and currently enrolled students may use this form inside the library to request a search for a periocial that is currently listed in USF Library Catalog as "Not Checked Out" or charged to missing but which is not on the shelf.
Problem Resolution The USF Tampa Library is offering this process as a means of systematically resolving problems and thus meeting your needs.
PRONTO Books & More USF Tampa faculty (only) may use this form to request that a circulating book or media item owned by the Tampa Library be delivered to their Campus Department mailbox. Up to 20 requests per week.
Proxy Request Form This form is used to request a proxy card for library use at the Tampa Library. Any Tampa faculty ,staff or student is eligible for a proxy card. IT MUST BE FILLED OUT BY THE SPONSOR.
Requesting New Materials or Rush Catalogiing
Forms here are for requesting items the library does not own, or requesting items listed as "in process" in the library catalog.
Rush Cataloging USF-Tampa faculty, staff, and currently enrolled students may use this form to "rush catalog" a Tampa item currently listed in the USF Library Catalog as "in cataloging" or "in process."
Suggest a Purchase Use this form to suggest purchases for the library collection.

Carrel Request Form Full-time faculty and Ph.D. candidates can request library carrels with this form.
Copy Services Copy Services provides copies of library materials including materials from Special Collections (books, periodicals, newspapers, and microfiche/film) to library patrons.
Presentation Practice Rooms Reserve a Media Resources multi-media room for course-related presentation practice. These rooms are equipped with large-screen projectors, computers with PowerPoint and other software, and laptop hookups. Multimedia rooms will be reserved for practicing presentations only, and may not be used for group study.
